At Aviva we are always looking for ways to improve our processes, so we want to introduce a new Policy Lapse Form.

 

Up to now all policy lapse requests or late lapse requests had to be sent individually by email or post. In the case of motor policies, the vehicle cert and disc could only be returned by post.

 

Our new Policy Lapse Request Form

 

To speed up this process, increase efficiency and reduce post for people working remotely, we’ve created a new Policy Lapse Request Form you can use with immediate effect. Please note, you can use the form for policy lapse requests for all products, with the exception of Motor Fleet.

 

Here’s how it works:

  1. Simply fill out the new Policy Lapse Form (see interactive PDF attached) for any policy you want to lapse and email it back to us.
  2. In the case of Motor Policies, make sure to write the word “VOID” on each Certificate of Insurance and Windscreen Disc with permanent marker, before scanning and emailing them, along with the Policy Lapse Form, to BrokerLapses@aviva.com
